چکیده

A 71-year-old woman with isthmic aortic coarctation underwent aortic valve replacement and coronary bypass. Before aortic clamping, end-to-side anastomosis to descending aorta was performed. After cardiac surgery, the graft was directed posterior to the inferior vena cava and anastomosed to the ascending aorta by using a side-biting clamp (Fig. 1). Fig. 1. Three-dimensional computed tomographic image (18-month post-operatively) showing isthmic aortic coarctation and ascending-to-descending aortic bypass performed using an 18 mm Dacron graft. The postoperative outcome was uneventful and the patient continues to do well 25 months postoperatively.

Aortic root aneurysm in an adult patient with aortic coarctation: a single-stage approach.

Coarctation of the aorta is a common congenital defect that may be undiagnosed until adulthood. Moreover, coarctation is associated with congenital and acquired cardiac pathology that may require surgical intervention. The management of an adult patient with aortic coarctation and an associated cardiac defect poses a great technical challenge since there are no standard guidelines for the thera...

Patent Extra-Anatomic Graft 12 Years after Ascending-Descending Aortic Bypass for Interpositional Graft Infection

A male car driver who sustained polytrauma in a road traffic accident when he was 25 years old, had emergency partial hepatectomy performed for liver laceration, followed by interpositional graft (18 mm woven Dacron) repair for transection of the proximal descending aorta via a left thoracotomy. He also had multiple fractures which were treated conservatively. His postoperative recovery was com...

Aortic coarctation in the adult: regarding a case and extra-anatomic bypass approaches.

The authors describe the operative technique approach and its post-operative course used in an adult patient with coarctation of aorta, in which it was not possible to perform the traditional correction with an end-to-end anastomosis with graft interposition. During the surgery it was necessary to make an extra-anatomic bypass: ascending-descending aorta with an enlarged left posterior-lateral ...

Involution of aneurysmal collateral arteries after correction of aortic coarctation.

A 47 year-old man with aortic coarctation had undergone extra-anatomic bypass through a left thoracotomy. He presented 10 years later with uncontrolled arterial hypertension due to residual aortic coarctation, graft obstruction and multiple collateral artery aneurysms between the subclavian artery and the aorta. He underwent extra-anatomic correction between the ascending aorta to descending ao...

One-stage surgical approach to coarctation of the aorta and ascending aortic aneurysm.

BACKGROUND Aortic coarctation in adults is sometimes associated with concomitant cardiovascular pathologies which require intervention. The optimal operative approach for such patients remains unsettled. An extra-anatomic aortic bypass from the ascending aorta to the descending aorta via median sternotomy allows simultaneous performance of repair of complex aortic coarctation and concomitant ca...
